hey every one!!

i hope your having as great of time as i am raising money for our team

 only one question...

where do i get the chaperone forms that the parents have to fill out...?

not the ones on the relay website.. the other ones

and  is it one chaperone for every fifteen kids right? because on the chaperone forms off of the relay websites say that it is five per child and i wasnt sure if i need more parents to tagg along with me...

also, does each child have to fill out a specifc form? or not because im still kind of fuzzy about that

 

and for luminaria... im doing the miramar relay at everglades and we have a ton of people we would love to honnor during luminaria and i dont know which forms to fill....

Hi,

 The chaperone forms and luminaria forms you can get from your ACS staff partner.

It is one chaperone for every 5 kids, so if you have 15 kids, you'll need 3 chaperones.

Each child and their parent needs to fill out a Youth Waiver form, which your ACS staff partner has as well.
